Most process safety studies produce excellent documents that very few people read. HAZOP worksheets run to hundreds of rows. QRA reports are dense with contours and frequencies. Both are necessary. Neither is something a shift supervisor can hold in their head.

Bow-Tie analysis exists to solve that problem. It puts an entire hazard scenario - causes, consequences and every barrier in between - on a single diagram that a non-specialist can read in a minute.

The anatomy of a bow-tie

The diagram is named for its shape. In the centre sits the top event - the moment control is lost. To the left, the threats that could cause it. To the right, the consequences that could follow. Barriers sit on both sides.

Why it earns its place

It communicates across levels

A board member, a plant manager and a panel operator can all look at the same bow-tie and take something useful from it. That is not true of a HAZOP worksheet, and it is one reason bow-ties have spread through safety-critical industries.

It makes barriers ownable

This is the most practically valuable outcome. Each barrier on the diagram can be assigned an owner, a performance standard and a verification method. Safety stops being an abstract property of the plant and becomes a list of specific things that specific people are responsible for keeping working.

Escalation factors are where the real insight usually is. A barrier that exists on paper but has an overdue proof test, an inhibited alarm, or an operator who has never been trained on it is not a barrier. Most organisations know this in the abstract. A bow-tie makes it visible for a specific scenario, which is much harder to file away.

It exposes uneven protection

Drawn honestly, a bow-tie often shows five preventive barriers on one threat and one on another, or a well-protected left side with almost nothing on the right. That asymmetry is difficult to see in a table of findings and obvious in a diagram.

It survives after the study

HAZOP reports tend to be consulted when something goes wrong. Bow-ties get used - in inductions, toolbox talks, management of change reviews and incident investigations. A study that stays in circulation is worth more than a better study that does not.

How it fits with HAZOP, QRA and SIL

Bow-Tie does not replace anything. It sits on top:

  • HAZOP identifies deviations and failure scenarios systematically. It supplies the threats.
  • QRA quantifies how likely and how severe the consequences are. It supplies the numbers.
  • SIL studies determine how reliable each instrumented barrier must be.
  • Bow-Tie assembles all of it into one picture and assigns ownership.

Where the underlying studies are weak, the bow-tie will be weak too - it organises existing knowledge rather than generating it.

Doing it well

  • Pick genuinely major scenarios. Bow-ties are for the handful of hazards that could cause a major accident, not for every deviation. Ten focused bow-ties beat sixty shallow ones.
  • Only count real barriers. A barrier must be effective, independent and auditable. "Operator vigilance" is not a barrier.
  • Be honest about escalation factors. A diagram with none has not been drawn properly.
  • Assign every barrier an owner. Unowned barriers decay quietly.
  • Keep it current. A bow-tie that does not reflect the plant as it is today is worse than none, because it creates false confidence.

For sites covered by Major Accident Hazard requirements, bow-ties also make it far easier to demonstrate to a regulator that hazards are understood and actively controlled - because the barrier list is exactly what an inspector wants to interrogate.

How many bow-ties should a plant have?

Bow-Ties are intended for major accident scenarios rather than every identified deviation, so the number is usually modest. A focused set covering the site's genuinely major hazards, kept current and actively used, delivers considerably more value than a large library that is not maintained.

Do we need a bow-tie if we already have HAZOP and QRA?

They serve different purposes. HAZOP and QRA analyse; Bow-Tie communicates and assigns ownership of barriers. Organisations that have both studies but struggle to translate them into day-to-day barrier management are usually the ones that benefit most from adding Bow-Tie.
